I. Summary of Position

Professional case management work that aids the expected mothers, new mothers, and young children of New Orleans. The Case Manager will interact with a diverse community, and promote the vision, mission, and values of Healthy Start New Orleans and the New Orleans Health Department.

II. Organizational Relationships

Supervisor: Case Manager Supervisor

Assignments received from: Case Manager Supervisor

  • NOHD program managers and initiative coordinators

  • Collaborative coalition leaders
  • The Office of Neighborhood Engagement
  • Chief Administrative Office

III. Duties and Responsibilities

A. Core Functions (provide at least 3):

  • Conduct client home visits, prenatal, postpartum, mental health and/or newborn assessments for well clients as well as participate in community outreach activities as needed.
  • Complete client intake procedures, data entry and documentation of all case management services.
  • Utilizing the Parents as Teachers Home Visiting Model, Case Managers will provide one on one parenting education.
  • Staff will attend PAT Foundational and Model implementation Training sponsored by PAT National Center.
  • Staff will conduct child developmental screenings and family centered assessments utilizing evidence-based tools.
  • Refer clients to critical healthcare, mental health and social service resources.
  • Advocate for clients to ensure they receive timely, culturally-competent services that address social determinants of health.
  • Facilitate client entry and linkage into various professional services, educational and workforce development programs.
  • Attend bi-weekly case conference and staff meetings, team meetings, educational training sessions and health fairs as needed.
  • Ensure all necessary information is thorough and accurate, all documents and evaluations are placed in both paper files and entered into the HSNO database system within specified timeframes in coordination with data entry personnel.
  • Develop or assists in the development of additional community resources as indicated by the needs of the clients.
  • Provide referrals for nutritional supplements, contraceptives, pre/post natal care, personal care, and baby items on an as needed or incentive basis. Help support operations of the Parent Resource Center.
  • Maintain knowledge of application rules, regulations, policies, laws and guidelines that impact personnel and administrative issues for the city and HSNO; develop internal controls that promote adherence to applicable state/federal laws, and program requirements; adhere to their licensure’s code of ethics (if applicable) and seek advice and guidance as necessary to ensure proper understanding.
  • Provide excellent customer service for both internal and external customers; approach all encounters with a warm, professional greeting and assists to problem solve whenever necessary; and recognizes when an issue needs further attention from a supervisor and inform management so that the problems can be addressed in a timely manner.
  • Ideal candidate should have experience in maternal and child health and possess a valid license in social work, counseling, or closely related field, or eligible to attain certification and or licensure within the first year of full-time employment with Healthy Start New Orleans.
  • May be required to attend specialized training such as Doula Birthing and Certified Lactation Counselor Training.
  • Other duties as indicated by Case Manager Supervisors and Healthcare & Social Service Manager

B. Additional Responsibilities (if applicable):

  • Will be expected to respond to public health emergencies.
  • Will require non-traditional working hours on occasion.

C. Physical Requirements, if any:

  • Occasionally required to lift 25 pound boxes

IV. Examples of Work (provide 3-5 examples)

  • Schedules and completes encounters with clients. Coordinate services and conduct consistent reassessment activities or when indicated by clients’ needs or lack of progress in achieving desired outcomes.
  • Facilitate discharge and/or transition from service systems and assigned client levels.
  • Participate in bi-weekly or monthly 1:1 reflective supervision and consultation with Case Manager Supervisor.
  • Provide referrals for nutritional supplements, contraceptives, pre/post natal care, personal care, and baby items on an as needed or incentive basis. Help support operations of the Parent Resource Center.

V. Qualifications and Skills

A. Minimum Qualifications and Skills Required by Department:

  • A Bachelor's degree from an accredited college or university. Official college transcript or original diploma must be presented within two weeks of application.
  • Four (4) years** of professional experience in case management, preferably with expected mothers, young mothers, and children.
  • **A Master's degree in Public Health, the social sciences, criminal justice or a Juris Doctorate may substitute for two (2) years of the professional experience.

Pay: $46,000.00 - $47,000.00 per year
