Job Description

Title: Case Manager

Department: Supportive Services for Veteran Families

Reports to: Housing Services Division Director

SUMMARY: Responsible for maintaining smooth and efficient office work flow, manage all incoming telephone calls and client requests for services, assessing client needs, data collection for intake to crisis resolution for qualified programs. Provide appropriate case management to clients.

DUTIES AND RESPONSIBILITIES:

1. Handle all incoming phone calls in a compassionated and professional manner.

Interviews identified clients to assess need linkage to other community resources.

2. Assessment: Assesses financial need, including expenses, financial resources and eligibility for various public services and supports. Assesses client's ability to follow through with a plan, including ability to make contacts with other agencies, such as

BSS or other network agencies, independently or whether assistance or direct advocacy is required. Assesses adequacy of client documentation for particular funding requirements. Screens for possible presence of additional problem areas, such as substance abuse that could require additional assessment and follow up, and informs supervisor.

3. Makes necessary contacts to verify client information. Maintains confidentiality of all client information and services provided at all times, in accordance with the policy of Catholic Charities and state and federal laws.

4. In straightforward cases, makes judgments about financial and material assistance to be received from Catholic Charities and through referral to other services. Records client information and services provided into data base. In complex or ambiguous cases, makes initial recommendation and obtains decision of supervisor.

5. Provides education, information and referral to ensure successful connection with other appropriate community or entitlement resources.

6. Counsels and motivates client through the community resource linkage process.

Empowers the client to act for themselves in advocacy and pursuing services, whenever possible.

7. When appropriate and necessary, provides direct advocacy for the client through direct contact with other agencies, creditors, etc.

8. Completes all paperwork and reports in a timely manner as required by supervisors and funding sources.

9. Participates in assigned meetings and trainings as related to program.

10. Participates in the implementation and ongoing development and continuous improvement of the program.

11. Ensures immediate and thorough communication to supervisor of program problems and provide recommended resolution to identified problem.

12. Performs other duties as assigned and location.

13. Maintains a positive work atmosphere by acting and communicating in a manner that fosters good relationships with customers, clients, external colleagues and management.

SUPERVISORY RESPONSIBILITIES: None

QUALIFICATIONS:

1. Knowledge of entitlement programs and program regulations, federal, state and community resources and expertise in navigating these resources.

2. Commitment to the mission of Catholic Charities.

3. Ability to relate directly but, compassionately with people in emergency situations

4. Must possess strong interpersonal and communication skills.

5. Personal transportation required.

6. Has a thorough understanding of all relevant community resources and eligibility or has immediate and ready access to this information.

7. Workable knowledge of office software, especially Microsoft Suite and Outlook.

EDUCATION AND/OR EXPERIENCE:

1. Bachelor's degree in human service field is preferred. High School Graduation required.

2. Two years of work experience in the human service field is required.

LANGUAGE SKILLS:

1. Bilingual preferred (English/Spanish)

2. Good oral communication skills required.

3. Ability to write clearly and proficiently for completion of documentation

MATHEMATICAL SKILLS:

Sufficient to complete statistical reports accurately. Ability to make calculations of client financial needs and resources.

REASONING ABILITY:

1. Ability to operate in multiple agency and community based work sites and make rapid assessments with moderate supervision.

2. Able to make judgments about assistance required from Catholic Charities, eligibility for other resources, resources available to the client and what judgments must be made by supervisors.

3. Must possess strong critical thinking and problem solving skills

4. Ability to rapidly identify potential for physical danger and develop a personal safety strategy.

PHYSICAL DEMANDS:

  • Sufficient mobility to move among work sites and access non-ground level sites.
  • Sufficient manual dexterity to complete necessary documentation and reports.

WORK ENVIRONMENT:

  • Contact with diverse clients and families in a variety of settings.
  • Subject to varying and unexpected situations, crisis and emergencies.
