Avenues Recovery Center of Lake Ariel is seeking to hire a Case Manager!

Avenues Recovery Center in Lake Ariel, PA is a unique community-based detox/residential treatment facility that's truly customized to the needs and unique circumstances of each individual. With our team of skilled medical and clinical professionals, Avenues Recovery offers first class treatment and recovery.

Primary Focus: The Case Manager/Client Care Advocate acts as a liaison between the multidisciplinary treatment team and any outside agencies an individual may be involved with or in need of services from. The client care advocates responsibility is to help each client effectively navigate needs outside of clinical services and ease the stress of external barriers so patients can maximize treatment.

Responsibilities Include:

Identify client urgent needs and resources to support smooth transition into treatment

• Meet client within 48 hours of admission
• Identify those directly related to client's recovery and secure releases.
• Ensure client basic personal needs are secured
• Collaborate with extended team to secure client needs and escalate communication to treatment team on all pertinent client information

Assist clients with legal documentation needed to remain in treatment

Review client account and ensure all financial agreements and benefits are in order

Ongoing Care Advocate Support

Care Advocate Role in Safe Discharge

Required Education

Graduation from High School or GED/Bachelor's Degree

Must be able to read, write, speak, and understand the English language

Required Experience

A minimum of 1-year experience in treatment/recovery industry preferred.

Licensure/Certification

Current and valid state issued Driver License

Valid CPR certification and First Aid
