Case Manager-Assisted Living Amber Court of Pelham Gardens
This is an excellent opportunity working for a recognized and respected leader in providing quality care to the elderly in an Assisted living environment. Potential for growth in our growing company. This is a dynamic position for a Case Manager.
The ideal candidate will have:
- A Bachelor's Degree in Human Services, Case Management, Social Work or the equivalent.
- Experience working with the adult or geriatric population.
- Assisted Living Experience a plus
- Good communication, computer and people skills.

As our Case Manager you will be assisting the residents in adjusting to their new home in our assisted living, troubleshooting any problems they may have, working with their families, dealing with hospitals, social workers and outside providers on their behalf.
