Position Details
Monday - Friday, Day shift; Occasional after - hours/ weekend

Position Summary
The purpose of this position is to provide case management services as are necessary to support the resident in maintaining independence of function and personal choice. Identify and evaluate the resident’s needs, interests, and strengths and the capability of the facility to meet those needs, prior to admission and then at least once every 12 months, oversee and coordinate a written Individualized Service Plan for each resident; in compliance with federal, state and local regulations and to promote person-centered care for optimal quality of care
Position Qualifications/ Requirements
Bachelors or Masters with major work in human service delivery and one year of full-time experience in the provision of services to a dependent adult population
Or
Associates degree with major work in human service delivery and 3 years full time experience in the provision of services to a dependent adult population
