Salary: $48,000 - $52,000 (DOE) (Non-Exempt)

Monday to Friday : 8:30 am to 5:00 pm

The Case Manager provides case management services to participants and their families with appropriate referrals and resources to assist them in sustainability. This includes coordinating support services to assist participants with independent living. The Case Manager is responsible for monitoring and maintaining documentation of client services within clients’ charts, ensuring compliance with agency policies and procedures, contractual and other related agencies’ standards and guidelines.

Education/Experience:

• Bachelors Degree preferred in a related field.
• Two years of experience working with at risk populations in a social service/mental health environment.
• Current California driver’s license, valid car insurance and registration.
• COVID-19 vaccination required. May request medical or religious exemption if applicable.
