Description

**This is a full-time position at 40 hours a week located at Lake Forest Hospital. **

The Case Management Coordinator reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

Responsibilities:

  • Prepares documents and manages data using NM software applications including Microsoft Office.
  • Manages calendar; sets meeting priorities; identifies objectives of meetings and identifies materials needed for meeting; exercises judgment to ensure appropriate parties attend meeting.
  • Screens incoming email, mail and telephone calls to assign priority to call and/or identify additional resources to manage callers need.
  • Screens walk-in clients within department guidelines to determine need to be seen by case management or social work. Clients with community resources questions should be referred to the Case Management social workers (847-535-6445/847-535-7524/847-535-7573/847-535-7441). Clients with billing questions should be referred to Patient Financial Services (847-535-6016).
  • Provides resource information regarding the following: nursing homes, help in the home, and other specific community resources as directed by professional staff.
  • Independently plans and executes activities such as room scheduling, signage, catering, support staff and equipment, and preparation and distribution of notifications, in support of projects and meetings as appropriate.
  • Copy or fax patient information, as needed, to outside agencies for continuity of care, as requested by professional staff.
  • Assists with creation and preparation of materials which are presented to senior management and Board-level committees and subcommittees, particularly power point presentations and printed materials.
  • Prepares, copies, mailings, and files documentation accurately and maintains security and confidentiality of highly sensitive information.
  • Participates in department meetings and committees.
  • Performs other assigned secretarial duties (including procuring medical records utilizing PowerChart/Cerner, and retrieving charts from Medical Records department).
  • Assumes responsibility for professional development by participating in workshops, conferences, and/or in-services.
  • Upholds the Northwestern Lake Forest Hospital policy on patient confidentiality.
  • Brings issues with legal, risk management, or ethical implications to the attention of the director in an expedient manner.
  • Monitors copy machines, fax machines, computers and telephonic equipment and reports malfunctions.
  • Maintains department schedule and secures coverage when staff are absent.
  • Maintains a sufficient inventory of all office supplies.
  • Participates in special projects under the direction of Director.
  • Performs other duties as assigned.

Additional Responsibilities:

  • Utilizes appropriate resources within the institution, division and department to manage and complete each assigned task and/or project.
  • Develops and maintains relationships with all customer and suppliers, especially vendors who support equipment within the department.
  • Provides support to Division Vice President as needed.
  • Acts in Secretary role for designated meetings and transcribes meeting content.
  • Conducts internet research and identifies relevant topicsbased upon assignment.

Qualifications

Required:

  • Associate's degree or equivalent 3-5 years experience in a business office with a strong emphasis on database management and customer service.
  • Knowledge of medical terminology.
  • Knowledge of Microsoft Office, PowerPoint and Excel.
  • Ability to organize and think independently.
  • Excellent communication and interpersonal skills, and ability to work in a team environment.
  • Ability to understand and execute oral and written instruction as requested.
  • Excellent organizational and prioritization skills, combined with initiative and good judgment.
  • Excellent interpersonal skills.
