Case Manager

Job Title: Case Manager

Department: Residential Programs

Pay range:$22-24

Summary: The residential Case Manager position will be assisting clients to obtain permanent housing. Case management services include, but are not limited to, case management assessment, individualized service planning, connection to employment services, health and wellness goals, addressing all service needs to stabilize clients in housing.

Essential Duties and Responsibilities include the following:

• Maintain accurate client data in the agency database, ETO and county database HMIS
• Provide strengths-based case management and service coordination services designed to assist clients in obtaining and maintaining stable employment and housing.
• Develop individualized service plans in collaboration with clients addressing short-term and long-term goals.
• Provide on-going case management support to assess progress and ensure goals are met or changed as needed.
• Provide services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate their own care.
• Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.
• Make warm handoff referrals to outside agencies that can benefit the client in reaching their individualized service plan goals.
• Assist with client intakes
• Attends and participates in all required meetings and training sessions.
• Perform UA testing for residents who are suspected of drug/alcohol use.
• Report any incidents or concerns to supervisor

Required Qualifications

• Employment Eligibility Verification.
• Negative tuberculosis test.
• Successful completion of background screening.
• Negative Pre-Employment Drug Screening Result
• Driving is an essential function of this position
• Must have Valid CA Driver's License
• Must be able to qualify for Good Samaritan Shelter insurance coverage

Education:

• Associates or Bachelor’s (Preferred), High School Diploma or GED (Required)
• Registered or Certified as a substance abuse counselor
• Have at least 2 years of experience working in social services, with populations in crisis
