Job description
Responsible for improving patient's level of engagement, overall experience with acute care services and ensuring appropriate utilization of in-network health system services. Initiate patient call-back and monitoring program for patients discharged from Commonwealth Health Hospitals; including inpatient, emergency department, and post-acute care discharges (i.e. long term care, skilled nursing home); bridging care and communication with patient's primary care physician and post-discharge and actively engaging the patient in overall healthcare management. BS in medical background preferred.Nursing, Social Work, Rehabilitation, Health Sciences or Pharmacy background is beneficial. Minimum of 2 years of clinical or healthcare environment experience. Effective communicator, knowledgeable of hospital based services and post-acute clinical resources. Project leadership and data analysis experience a plus.
Knowledge of managed care, patient experience performance, quality measures and value based healthcare is preferred.
