Summary: The RN Case Manager will coordinate care for high risk, complex clients to access needed health services and complementary social services. The RN Case Manager will coordinate services and community support to achieve improved health outcomes for clients.

Areas of Responsibility:

Patient Care

  • Work closely with patient’s care team and providers to enhance patient experience, understanding, and involvement in their health care
  • Review daily referrals; including clients admitted to hospital care in order to review client’s discharge plan, medical follow up, and psychosocial needs
  • Assist client in establishing individualized health goals, identifying potential problems/barriers; provide health education resources. Provide advocacy for client and monitor client’s progress. Coordinate logistics for health goal adherence
  • Assist in timely scheduling of primary care and other medical appointments to support client health; may attend appointments with client
  • Review changes to medication regimen and medication use
  • Clarify medical terminology and assist patient in understanding current health status and subsequent goal setting tools that will be utilized to encourage patient improvement

Community Support

  • Link individuals to application assistance for health coverage programs. Perform client benefit enrollment in areas where certified/approved
  • Maintain good working relationship w community health partners by working as a liaison with other agencies and the community; participate in the Task Force for the Homeless case manager coordination group
  • Coordinate with grant managers and program managers to develop Telemedicine and other innovative programs

Education/Experience:

  • Bachelor’s Degree in Nursing or related field with three (3) years’ experience that can be demonstrated to be applicable to the duties listed in the job description

  • Modern practices and principles of method disease counseling

  • Homeless resources and community partners that work with homeless clients
  • Basic management practices and principles

  • Grants management and reporting
  • Universal precautions and infection control practices
  • Pertinent Federal, State and local laws, codes, and regulations regarding health care

  • Provide administrative and nursing orientation to staff
  • Assess and evaluates patients’ responses to service provided, treatment, and medication
  • Demonstrate leadership skills and a desire to continually enhance nursing practice and performance
  • Maintain confidentiality of patients
  • Prepare clear and concise reports
  • Respond to requests and inquiries from the general public
  • Work independently in the absence of supervision, utilizing prioritization and delegation techniques for time optimization
  • Communicate clearly and concisely, both orally and in writing
  • Apply basic time management and patient care coordination skills to effectively provide nursing services
  • Establish and maintain effective working relationships with those contacted in the course of work, including service representatives and the general public
  • Plan and organize multiple work tasks, problem-solve and analyze information toward sound decisions

Licenses and Certifications:

  • Possession of an appropriate, valid California Registered Nursing License
  • Possession of, or ability to obtain, an appropriate, valid California driver’s license
  • Possession of, or ability to obtain, an appropriate, valid BLS certificate
  • Candidate must be able to successfully meet PHC’s credentialing and privileging requirements

Language Skills: Strong verbal and written communication skills with demonstrated ability to communicate professionally at all levels. Must speak, read, write and comprehend English. Bilingual in English and Spanish, written and verbal required.
