Conduct comprehensive case management activities designed to support and facilitate at risk pregnant clients access to health care and other appropriate services.
WHAT YOU WILL NEED:
- MASTERED DEGREE PREFERRED
- DEMONSTRATE A MINIMUM OF 8-10 YEARS IN THE FOLLOW IN AREAS OF CAPABILITIES:
- Manage a staff
- Facilitating program start-up
- Manage programs
- Community outreach and community engagement
- Grant management
- Human service and health care experience
