The Wholeness 2 Peace Care Services, Direct Support Program (DSP) will provide appropriate services based on an understanding of assigned families; and will assist them in acquiring or improving skills in child and home management, interpersonal interactions, job development/placement, living skills/social skills, parent education/training, crisis intervention and obtaining respite care or other resources in the community.The primary mode of service delivery is home or school based, and also at the Agency’s facility.

The Behavior Coach (BC) will display emotional stability, mature judgment, sincere interest in children and families, and acceptance of families as partners with strengths.

Education and Experience

The BC will have a B.A. or Associate Degree in Social Work, education or related field and 2 years experience. Experience in working with diverse populations is desirable.The Behavior Coach /Behavioral Health Tech are skillful in organizational skills and record keeping, along with an adequate knowledge of community resources in Maricopa County.

Responsibilities and Duties

• The BC will complete intake assessments, determine client needs and provide appropriate services to clients.
• The BC will monitor and record client progress, keep accurate and timely case contact notes, data reports, follow up with families as needed and discharge summaries.

Complete all assignments in a timely manner, attending all Child and Family Team staffing as required, and provide all needed services in a culturally competent manner that shows respect and takes into account cultural differences, norms and customs.

• Assist individuals in developing and practicing the skills defined in their Treatment Plans or other activities of interest including religious, educational, or vocational to provide support and encouragement.
• Coordinate with the DSP director on all activities regarding clients meetings and groups.
• Ensure each client receives appropriate care in accordance with the client's treatment plan, under the supervision of the DSP Director.
• Communicate to the Director all issues, concerns, activities, progress, and regression of Wholeness 2 Peace Care Services LLC and its clients.
• Attend staff meetings and other appropriate staff meetings and educational activities.
• Maintain strict confidentiality of guidelines regarding all clients, conversations, and referrals.Employment RequirementsThe BC must pass the Department of Public Safety fingerprint check, complete the Agency’s Orientation and all other required training. Have a valid Arizona Driver’s license, and auto insurance.Pursuant to Arizona Administrative Code R9-10-306 CHAPTER 10 of the Dept. of Health Services, personnel must be at least 21 years of age.

The BC must pass the Department of Public Safety fingerprint check, complete the Agency’s Orientation and all other required training. Have a valid Arizona Driver’s license, and auto insurance.Pursuant to Arizona Administrative Code R9-10-306 CHAPTER 10 of the Dept. of Health Services, personnel must be at least 21 years of age.
