Position Overview

The Assistant Director of Hospital Engagement is a member of our business development team and works with our directors of hospital engagement in assisting, patients and their families with the transition from facility to home based care. The Assistant Director of Hospital Engagement uses his or her knowledge of the referral process to work with the Aveanna Healthcare locations to ensure a positive outcome for the clients, families, and case managers.

Essential Job Functions


  • Work with designated Director of Hospital Engagement (DHE) to create and carry out a specific sales plans and market development plan in with company goals and objectives
  • Organize and upkeep referral pipeline and data in real time
  • Rotate through office locations to assist with staffing and help to ensure maximum fill rate at SOC.
  • Track LMN/WL to ensure quick authorization process and expedite SOC (revenue generating, prevents lost cases especially in the outpatient setting)
  • Follow-up with and re-engagement of former referrals
  • Prospect in designated outpatient locations
  • Assist in streamlining communication between accounts, DHEs, offices locations, and patients/patient caregivers
  • Manage patient calendars in advance of discharge
  • Track patient admission and retention data

    • Respond to patient questions and concerns; assess patient needs, status, and tolerance issues, trouble shoot and make recommendations as indicated or requested.
    • Coordinate efforts with and provide pertinent updates to Payer Relations team throughout patient onboarding process
    • Respond and follow up with branch requests, including insurance issues, medical eligibility, and clinical information for authorizations.
    • Assist with special projects such as specific patient needs in the community, ongoing training requests, hospital discharge process

Requirements

  • Bachelor’s degree in Business, Healthcare, or related field; equivalent work experience may be substituted in lieu of degree
  • Must have a valid driver’s license and driving record that meet company standards
  • Proficient in Microsoft suite of products including Outlook, Word and Excel
  • Ability and willingness to travel locally
  • Active and unencumbered nursing license

Preferences

  • Recent Home Care/Private Duty Experience preferred
  • Previous experience with Children's Hospital a plus
