Job description
HeartCorps members will help accelerate the implementation of clinical and community interventions that improve cardiovascular health, educate and inspire community members to adopt healthy behaviors, and gain an expanded and transferrable set of skills and experiences that equip them for future public health roles. HeartCorps members will participate in extensive training, coaching, and one-on-one supervision, building a transferrable skill set that can be activated in rural communities. Service members will learn from both AHA leaders and their host site leaders, and they will be exposed to a variety of health-related professions and opportunities. Upon completion of service, HeartCorps members will be better equipped to pursue future opportunities in public health or healthcare. To address the disproportionate rates of uncontrolled hypertension and other cardiovascular diseases in rural communities, HeartCorps members will be responsible for facilitating the adoption and implementation of sustainable systems changes and support strategies necessary to achieve blood pressure (BP) control. Example activities may include BP self-monitoring initiatives, increasing adoption of evidence-based BP measurement and treatment practices, facilitating BP screenings, connecting health centers with social platforms to facilitate ongoing screening and referral for social needs, establishing local support for patients/community members in navigating the care or resources they need, and more. Service members will also lead local collaborations beyond their host site intended to increase awareness of public health needs and further grow the public health workforce. Members may plan and implement local engagement events – such as days of service or community conversations – and will engage local high schools and colleges in building ambassador clubs to further support health promotion work and seed interest in public health professions.

Member Duties : Support blood pressure self-monitoring initiatives Facilitate adoption of evidence- based blood pressure measurement practices. Enable ongoing BP, nutrition security, and tobacco screening and referral to needed services Build community capacity to support healthy living and skill building including organizing, connecting, and/or delivering community training and education opportunities. Engage local and social media in blood pressure control and healthy living promotion. Establish local support for patients/community members in navigating the care or resources they need. Plan and implement local engagement events – days of service, town halls or community conversations – to raise awareness of blood pressure, cardiovascular health, and related issues and engage the community in supporting one another in key strategies for management.
