Acclaim Multi-Specialty Group and JPS Health Network has an opportunity for an experienced Advanced Practice Provider (PRN) to join the Community Health/Primary Care float team. The float team supports 11 JPS Patient Centered Medical Homes throughout Tarrant County. The clinics are comprised of fantastic teams of Family Medicine, Internal Medicine, Pediatric, and Advanced Practice Professionals.

As the county health network and Level-1 Trauma Center, JPS has been serving the residents of Tarrant County for over 100 years. In addition to providing high-quality care, JPS is also an urban teaching hospital with a long history of training physicians and medical students. JPS offers 18 residency and fellowship programs, including the largest family medicine residency in the nation. JPS educates in tandem with the University of North Texas Health Science Center, Texas College of Osteopathic Medicine, and Texas Christian University Medical School.

Schedule - PRN. Must be willing to work a flexible schedule Monday - Saturday (8, 10 or 12 hour shifts) - days and shifts can vary.

Job Summary:

Working under the supervision of a physician, the Advanced Practice Provider is responsible for administering and interpreting all aspects of patient care, including diagnosis, treatments and consultations within the outpatient setting. This position will also educate patients about preventative care and prescribed treatments; may also conduct physicals, order tests and serve as a patient's healthcare provider.

Duties and Essential Job Functions:
1.Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.
2. Reviews patient medical charts to assist physician with diagnosis, treatment and care management.
3. Makes appropriate patient referrals as necessary.
4. Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
5. Provides care education to patients and families.
6. Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
7. Maintains a collaborative environment with providers, clinical staff, and support staff.
8. Supports academic, educational and research endeavors.
9. Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
10. Serves as a resource to the community.
11. Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:

  • Master’s Degree in Nursing from an accredited school of nursing OR Graduate of an accredited Physician Assistant Program
  • Minimum of 2 years working independently as an advanced practice provider in a primary care setting

Required Licensure/Certification/Specialized Training:

  • Must have current Texas Physician Assistant license or Texas APRN license with FNP certification
  • Valid, unrestricted DEA
  • Basic Life Support (BLS) certification
