Advanced Practice Provider PA or NP - Lakewood Medical Center

The Practice:
Lakewood Medical Center has been providing quality medical care to the Arvada/Wheat Ridge/Lakewood area for over 20 years. We are seeking an Advanced Practice Provider to join our clinic and help serve our diverse patient population. We are an independent, physician owned primary care practice located in Lakewood, Colorado with a focus on internal and family medicine. Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Position:
  • Performs a comprehensive patient health assessment, including health history, physical exams and preventive screening.
  • Orders, administers and analyzes diagnostic tests.
  • Performs advanced technical procedures.
  • Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
  • Maintains accurate, legible and confidential records.
  • Trains patients and families on managing illnesses or injuries and disease prevention.
  • Develops and maintains a good working relationship with the healthcare team.
Qualifications (Education/Experience/Knowledge/Skills/Abilities):

  • Possess current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing OR valid licensure as a Physician Assistant from the Colorado Medical Board
  • Valid Prescriptive Authority in the State of Colorado
  • Current Drug Enforcement Administration (DEA) certification
Job Schedule: Full Time
Shift: 8-4 Days
Workdays: 4 or 5 days per week; usually Monday through Thursday or Friday, occasionally Saturday
Travel: 0 %
Salary Range: $95K- $135K depending on experience
