Acclaim Multi- Specialty Group, a partner with JPS Health Network is seeking a motivated an ambitious Nurse Practitioner (NP) or Physician’s Assistant (PA) to join the primary care team at the Family Health Center located on the 4th floor of JPS Hospital in Fort Worth, TX. FHC serves as the primary residency clinic for the #2 Family Medicine Residency in the United States, providing primary care, women’s health, office procedures, obstetrics, clinical pharmacy, pediatrics, and refugee care. Located within the JPS hospital system, full radiology, laboratory, pharmacy and emergency services are all within the same building.

As the county hospital system, JPS has been serving the residents of Tarrant County for over 100 years. This is a great opportunity to treat complex medical issues through team based collaborative care with residents and faculty, model good practices, and teach the next generation of full-scope family physicians through providing compassionate, quality care to those patients that need it most.

Schedule –Schedule – Monday – Saturday 7-7. Most Advance Practice Providers (APPs) work 3 days per week with a Saturday that alternates every other week.

Job Summary:
The Advanced Practice Provider practices under the direct supervision of the supervising physician and is responsible for assisting with all aspects of patient care, including diagnosis, treatments and consultations within the outpatient setting. This position will also educate patients about preventative care and prescribed treatments; may also conduct physicals, order tests and serve as a patient's healthcare provider.

Duties and Essential Job Functions:
Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.
Reviews patient medical charts to assist physician with diagnosis, treatment and care management.
Makes appropriate patient referrals as necessary.
Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
Provides care education to patients and families.
Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
Maintains a collaborative environment with providers, clinical staff, and support staff.
Supports academic, educational and research endeavors.
Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:
Master’s Degree in Nursing from an accredited school of nursing OR Master’s Degree in Physician Assistant studies
Minimum of 2 years of experience working independently as a Primary Care Nurse Practitioner or Physician Assistant.

Required Licensure/Certification/Specialized Training:
Must have current Texas Physician Assistant license or Texas APRN license
Basic Life Support (BLS) certification
Advanced Cardiac Life Support (ACLS) certification
