  • Work directly with the Facilities’ leadership to facilitate the admissions process
  • Work closely with hospital discharge planners, to affect a smooth, seamless transfer for individuals needing short or long term services into appropriate Grace or Grace affiliated programs
  • Conduct patient assessments to identify patients for potential admissions into the system
  • Work closely with patients and their family members to educate them on the benefits of Grace and Grace affiliated programs and services
  • Gather pertinent information regarding referral for the Admission Team to determine feasibility for admission; works closely with the Admission Team/individuals at each program
  • Manage the admissions process as an ambassador for patients who meet the admissions requirements; acting as a liaison between the consumer and the program
  • Communicate on an ongoing basis with the appropriate director/contact person concerning potential admissions and vacancies
  • Conduct one on one contact with potential referral sources, work to expand the number of referral sources
  • Maintain current referral sources through positive relationship development
  • Meet or exceed occupancy goals
  • Will assist other members of the social service department to complete any and all social service tasks as needed by Grace or Grace affiliated programs
  • Collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written communication
  • Maintain confidentiality of all patient and/or employee information to assure patient and/or employee rights are protected
  • Demonstrate good interpersonal skills when working or interacting with physicians, patients, their families, other providers and other staff members
  • Additional duties as assigned
